Faith Formation

Our Lady of Consolation Faith Formation Ministry, drawing on the power of the Holy Eucharist, is grounded in the Good News of our Lord and Savior Jesus Christ. Our program is here to help our families grow in holiness and faith by teaching Sacred Scripture, Sacred Tradition, relying on the Catechism and being obedient to the Magisterium of the Catholic Church. With great devotion to the Blessed Virgin Mary we are led to grow through Christ, with Christ, in Christ, in union with the Holy Spirit – sharing the faith, loving, and serving others.

  I hereby authorize Our Lady of Consolation permission to use my likeness in a photograph in any and all of its publications, including but not limited to all Our Lady of Consolation's printed and digital publications. I understand and agree that any photograph using my likeness will become property of Our Lady of Consolation and will not be returned.
  
  I acknowledge that since my participation with Our Lady of Consolation is voluntary, I will receive no financial compensation.
  
  I hearby irrevocably authorize Our Lady of Consolation to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing Our Lady of Consolation's programs or for any other related, lawful purpose. In addition, I waive the right to inspect or approve the finished product , includng written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photgraph.
